In a medium saucepan, mix the granulated sugar, unsweetened cocoa powder, whole milk, and unsalted butter.

Place the saucepan over medium heat and bring mixture to a boil, stirring constantly.

Once boiling, allow to cook for 5 minutes undisturbed.

Remove from heat and add vanilla extract and chocolate chips, stirring until chocolate is completely melted and mixture is smooth.

If desired, gently fold in chopped nuts until evenly distributed.

Pour the fudge into a greased 8×8 inch pan, spreading evenly. Allow to cool at room temperature until fully set.

Once firm, cut fudge into squares using a sharp knife.
